  I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan.  A 65 year old male resident of veliminedu, chityala who is maestri  by occupation, was brought to the hospital on 16th of April 2021 with chief complaints of swelling in both the legs since 14 days, decreased urinary output since 10 days and altered sensorium since 2 days  History of presenting illness : The patient was apparently asymptomatic 14 days ago, he then developed swelling in both the legs which was insidious in onset, gradually progressive, grade 3 pitting oedema, aggravated on rest and relieved by walking.   1601006089 : short case Final practical  This is an online E log book to discuss our patients de identified data shared after taking his /her/guardians signed consent form  35 year old male patient , resident of ramachandrapuram works as a daily wage labourer, since 20 years he is a chronic smoker (1 pack of beedi every 2 days ) and a chronic alcoholic(90 ml daily ) . He presented with a 3 month history of painless papules with erythema that started initially over the face ,then it gradually extended to upper limb ,lower limb and trunk  No discharge  Mild loss of sensations  No genital lesions  He had soap and topical cream available and some oral drugs taken from the nearby medical shop  He used this remedy for 2 months , some lesions regressed over time ,but few persistent lesions were noticed on hands ,leg ,trunk ,earlobe ,nose  Then the patient developed blackish skin discoloration 14 days  ago over hands and feet with skin peeling, cracking, ulcer formation associated with burni
